Office Policy Regarding Cost of Copying Patient Records


WHEREAS the Florida Administrative Rule 59R-10.003 states that all patients who request a copy of their medical records to be released to themselves will be held to the following provisos:
  1. Any person licensed pursuant to Chapter 458, Florida Statutes, required to release copies of patient medical records may condition such release upon payment by the requesting party (the patient) of the reasonable cost of reproducing the records.
  2. Reasonable cost of reproducing copies of written or typed documents or reports shall not be more than the following:
    • For the first 25 pages, the cost shall be $1.00 per page.
    • For each page in excess of 25 pages, the cost shall be 25 cents per page.
  3. Pursuant to the Florida Statutes, Chapter 395, there is a charge for your medical records at a rate of $1.00 per page for paper copies, $2.00 per page for microfilm copies and $1.00 per year search fee.
THEREFORE, you will need to reimburse the Drug Study Institute for the designated per page amount authorized by the above notes statute.
FURTHERMORE, please allow ten working days for the photocopies to be obtained through our office and/or outside photocopying service as deemed appropriate.
HENCEFORTH, it remains the office policy of Drug Study Institute to charge the allowable $1.00 per page for the first 25 pages and 25 cents per page for each additional page of your medical records that you are requesting for your own personal use.
IT REMAINS our office policy to forward a copy of your records to your physician at no cost.
